Time... It is modern man’s greatest enemy and also his most precious treasure. We are trying to catch up with everything; we want every job done "urgently," and every solution provided "immediately." But how realistic is this passion for speed when it comes to our health, and specifically, our smile?
Imagine waking up one morning with unbearable pain or a loose front tooth. You go to the dentist, and that inevitable verdict is delivered: "This tooth needs to be extracted." At that moment, your world might feel like it’s collapsing. A single fear scenario plays on loop in your mind: "Am I going to walk around toothless now? How will I go out in public? Will I have to hide my mouth during meetings or dinners?"
In the past—during the "golden age" of classical implantology—the answer would unfortunately have been, "Yes, you will have to manage for a while." The tooth would be pulled, you’d wait. The implant would be placed, you’d wait another 3 to 6 months for the bone to heal. It was a grueling process lasting nearly half a year, often spent struggling with removable dentures.
But the year is 2026. Technology has revolutionized the field. Now, in Gaziemir, Izmir, it is possible to walk into Denta Perla Diamond Dental Clinic with a missing tooth and walk out (provided conditions are met) with fixed teeth on the very same day, or at the latest, within 48 hours.
In medical literature, we call this "Immediate Loading." Our patients simply call it a "Miracle." Let’s put this treatment, considered the Formula 1 of the dental world, on the table. Who is it for? Is it safe? And what are the strict protocols we follow at Denta Perla Diamond to ensure success?
To tell the truth, it is not magic; it is pure physics and biology. In the classical method, the implant is placed in the bone, and the gum is stitched over it. It is left to "sleep" for months to fully fuse with the bone (osseointegration). This was because, in the past, implant surfaces were not as advanced as they are today, and bonding took time.
However, the Premium Segment implants (like Straumann, Nobel Biocare, etc.) that we prefer to use today at Denta Perla Diamond are produced with special technology (hydrophilic surfaces) that literally attracts bone cells. Furthermore, the thread designs of these implants are so aggressive and retentive that the initial gripping power—what we call "Primary Stability"—reaches immense levels the moment it is placed in the bone.
This is exactly what "Immediate Loading" is: If, at the very moment we place the screw, it grips the bone like a rock (exceeding a certain torque value), we say, "Why wait?" And we immediately attach a temporary but fixed tooth onto that implant. So, you do not remain toothless while your implant fuses with the bone; you go through that healing process with your tooth already in place.
We wish we could do it for everyone... But in medicine, the principle of "first, do no harm" applies. Immediate loading is an advanced protocol, and patient selection is critical. At our clinic in Gaziemir, we apply a very rigorous screening process, not to discourage our patients, but to protect their health.
For Immediate Loading (Teeth in a Day) to be performed, the following conditions must be met:
Bone Quality (Hardness): Think of an implant like a wall plug (dowel) being driven into a wall. If the wall is like concrete (Type D1-D2 bone), the plug grips immediately, and you can hang a shelf on it right away. But if the wall is like drywall (Type D3-D4 soft bone), the plug will just spin. We do not perform immediate loading in soft bone; we wait.
Torque Value (Newton): There is a tightening force measured by our device while placing the implant. If the implant locks into the bone with a force of 35-40 Newtons (Ncm) or more, we proceed with immediate loading. If it falls below this, we do not take the risk.
ISQ Value (The Ostell Device): At Denta Perla Diamond, we do not work on "guesswork." We take measurements with digital devices that measure the frequency of the implant's hold in the bone (Resonance Frequency Analysis). If the value is above 70, it’s a "Green Light." Below that, the decision is to "Wait."
Smoking and Grinding (Bruxism): Immediate loading is risky for patients who smoke a pack a day or grind their teeth at night. The excessive load or poor circulation on a freshly placed implant can lead to failure.
Let’s say you flew in from abroad or you have limited time in Izmir. You came to us. What awaits you in that chair?
First, we take your 3D Tomography (CBCT). This is our roadmap. We identify the hardest, most solid parts of the bone. We simulate on the computer at what angle we need to place the implant to achieve the highest retention.
Under local anesthesia (or sedation if you are anxious), if there is a tooth to be extracted, we remove it atraumatically (without damaging the bone). In the same session, into the same socket, we place the implant. This is called an "Immediate Implant." The surgical time is usually only 15-20 minutes per implant.
The implant is in. Your doctor takes the measurement with the torque wrench or the ISQ device. If the numbers are perfect, we give you the good news: "Congratulations, you are leaving with your tooth today."
Your impression is taken with digital scanners. In our laboratory or using our in-clinic milling machines (CAD/CAM), a "Temporary Fixed Tooth" is produced to be screwed onto the implant. This tooth is aesthetically perfect but functionally "taken slightly out of contact." Meaning? When you smile, it looks great, but when you bite down, it does not fully crash against the opposing tooth. Why? Because we don't want excessive load on the implant while it is healing.
About 2-3 months later, when the implant has fully integrated with the bone, we remove that temporary tooth and replace it with your fully functional Zirconia or E-max porcelain tooth that you will use for a lifetime.
Why do we take the risk (as it is technically more demanding for the physician) and apply this method? Because patient comfort is everything to us.
Psychological Comfort: Arriving toothless and leaving toothless is traumatic. Leaving with teeth is confidence. You don't fall behind in your social life or work for even a single day.
Gum Shaping (Pink Aesthetics): In the classical method, while waiting with the implant covered, the gum flattens out. Then, when the tooth is placed later, it doesn't look natural. In immediate loading, the temporary tooth we attach presses against the gum, giving it a "natural tooth emergence profile." When your final tooth is placed, no one can tell it is an implant.
No Second Surgery: In the classical method, you need to cut the gum again 3 months later to uncover the implant. Immediate loading skips this stage entirely.
This is the most critical point. Our patients often think that once the "fixed tooth" is in, they can eat anything immediately. No, you cannot.
For our patients who undergo Immediate Loading (Same Day Teeth), the "Soft Diet" rule applies. For the first 6-8 weeks:
No biting into apples.
No tough meat, nuts, or crusty bread.
Foods that can be easily mashed with a fork like soup, pasta, mashed potatoes, fish, and eggs are allowed.
If you do not follow this rule, you will move the implant that has not yet fully fused with the bone and cause it to fail. This is a partnership. We perform the perfect surgery, and you protect that baby (the implant) for 2 months.
Yes, there are many places doing implants in Izmir. But for a delicate procedure like Immediate Loading, which requires high technology and experience, choosing the right clinic is vital.
Airport Location: Time is money for our patients coming from abroad. With our location in Gaziemir, you can get off the plane and be at our clinic in 10 minutes, get your treatment, and move on to your hotel.
Stock Power: To perform immediate loading, you need to have every size and diameter of implant and every type of abutment (connecting piece) on hand at that exact moment. Denta Perla Diamond’s massive inventory reduces waiting time to zero.
Laboratory Integration: Strong laboratory support is essential to produce the temporary tooth on the same day. Thanks to our in-house digital workflow, your tooth is ready within hours.
Toothlessness is not a destiny. And "you just have to wait" is no longer the only option. If your bone structure is suitable and your general health is good; with the expert physicians and state-of-the-art devices of Denta Perla Diamond, you can walk out of the door you entered toothless in the morning with a brand new smile in the evening.
You are at the right address for treatments that keep up with the speed of time and your life's tempo. Come, let's take your 3D scan and see if your bone is ready for this speed.
